MI Choice Waiver: Eligibility, Services & How to Apply

Michigan's MI Choice Waiver: A Dedicated Guide to Getting Care at Home in 2026

Nobody plans for the moment things change.

One week, life is normal. Next, someone you love cannot manage alone anymore. Not because they stopped trying, but because the body has its own timeline. And it does not ask for permission.

Maybe it started with a fall. Or a missed meal. Or a phone call from a neighbor who said, quietly, that she had not seen your father outside in a while.

And now you are sitting at a kitchen table—or maybe a laptop at midnight—typing words like "Michigan home care" and "how to keep Mom out of a nursing home" and wondering if there is actually a real answer out there.

There is.

This is your MI Choice Waiver dedicated guide—written in plain English, built for real families, and designed to give you the clearest picture possible of one of Michigan's most powerful and underused Medicaid programs. By the time you finish reading, you will know exactly what the MI Choice Waiver is, whether your family qualifies, how the application works, and what to do if there is a waitlist.

📌 Quick Answer :

What is the MI Choice Waiver? The MI Choice Waiver is a Michigan Medicaid program that pays for home care services for seniors and disabled adults who would otherwise need to live in a nursing home. It lets people stay in their own homes and still receive the care they need — at little or no cost to them.

What Is the MI Choice Waiver Program? (Let's Keep It Simple)

The MI Choice Waiver Program is Michigan's way of bringing nursing-home-level care right to your front door. It is part of what the federal government calls Home and Community-Based Services (HCBS) — a national effort to let people get serious care support without being forced into a facility.

Michigan's version is called MI Choice, and it has been helping residents live independently. It is overseen by the Michigan Department of Health and Human Services (MDHHS), which contracts with about 20 regional waiver agencies—many of them Area Agencies on Aging (AAAs)—to deliver services across the state.

Think of it this way. In the past, if you needed serious help with daily tasks — bathing, eating, moving around — a nursing home was often your only real option. The MI Choice Waiver changed that. It lets Medicaid pay for those same services at home, in an adult foster care home, or in a community setting instead.

⚠️ Important: MI Choice is NOT guaranteed to everyone who qualifies. There are a limited number of spots — about 20,543 per year statewide. When spots fill up, a waitlist forms.

We will talk about that — and what to do — later in this MI Choice Waiver dedicated guide.

Who Qualifies for the MI Choice Waiver? (The 3-Part Test)

This is the question every family asks first. And it is a fair one. So let us break it down into three simple parts.

Part 1: Age and Where You Live

To be eligible for the MI Choice Waiver, you must:

  • Live in Michigan
  • Be 65 years old or older, OR
  • Be 18 or older with a qualifying physical disability
  • Be at risk of needing nursing home care — meaning without help, you would likely need to move to a facility
Part 2: Medical Need — The Level of Care Test

This part trips people up. You do not just need a doctor's note. You need to meet what Michigan calls a Nursing Facility Level of Care (NFLOC).

A trained team—usually a registered nurse and a social worker—will visit your home and use an online tool called the Level of Care Determination (LOCD) tool to officially assess your care needs. They look at three main areas:

  • Daily Activities (ADLs): Can you bathe yourself? Transfer from bed to a chair? Eat on your own? Use the toilet safely?
  • Mental and Cognitive Ability: How is your memory? Can you make basic daily decisions? Can you communicate your needs?
  • Behavior: Do you wander? Refuse care? Have difficulty managing emotions or behaviors?

You also need to require supports coordination—which is basically a care manager — plus at least one other service from the waiver.

💡 Quick note: Being diagnosed with Alzheimer's or dementia does NOT automatically qualify you. What matters is how the condition affects your ability to do daily tasks. The LOCD assessment measures that directly.

Part 3: Financial Limits — Income and Assets in 2026

Now let us talk money. The MI Choice Waiver uses Michigan Medicaid's financial rules. Here is what the limits look like in 2026:

What It Covers 2026 Limit
Monthly Income — Single person Up to $2,982 per month
Monthly Income — Married (each spouse) Up to $2,982 per month each
Asset Limit — Single person $9,950
Asset Limit — Married (applicant spouse) $9,950
Community Spouse Resource Allowance (CSRA) Up to $162,660 (50% of shared assets)
Minimum Spousal Income Allowance $2,643.75 / month (July 2025 – June 2026)
Maximum Spousal Income Allowance $4,066.50 / month
Home Equity — exempt up to $752,000 (if it is your primary home)

Good news: Not everything counts. Your home, one vehicle, household furniture, and personal belongings are usually not counted toward the asset limit.

⚠️ 60-Month Look-Back Rule: If you give away money or sell assets for less than fair market value within 5 years of applying, Medicaid may delay your benefits. This is called the Look-Back Period. Do not do this without professional guidance.

What If You Are Over the Income or Asset Limits?

Being slightly over the limit does not always mean you are out. There are legal ways to plan around it.

Michigan does not allow Irrevocable Funeral Trusts. But it does allow Irrevocable Prepaid Funeral Contracts — where you prepay funeral costs and that money stops counting as an asset. Medicaid-Compliant Annuities are another tool that can turn a lump sum into a monthly income stream.

One important thing: Michigan's Spend-Down Program does not work for MI Choice. Because MI Choice covers all your care costs, there are no remaining medical bills to spend down against. You can check other options on Benefits.gov or speak with a qualified Medicaid planner.

What Does the MI Choice Waiver Actually Pay For?

This is where the program really shines. The MI Choice Waiver covers a wide range of services — far more than standard Medicaid home care. Everything is based on your personal care plan, built around your specific needs.

For the full official list, you can also download the MI Choice Participant Handbook from the MDHHS website. Here is what may be covered:

  • Community Living Supports: Help with bathing, personal hygiene, eating, meal prep, grocery shopping, housekeeping, non-emergency rides, and dementia care
  • Adult Day Health Care: Supervised group care in a community setting — minimum 4 hours a day, with transportation often included
  • Supports Coordination (Case Management): Your dedicated care navigator who builds your plan and connects you to services
  • Home-Delivered Meals: One to two meals per day — including meal kit services and grocery delivery fees
  • Respite Care: Short-term care that gives your primary family caregiver a real break
  • Home Modifications: Ramps, grab bars, wider doorways, wheelchair-accessible bathrooms
  • Nursing Services: Intermittent skilled nursing visits in your home
  • Private-Duty Nursing / Respiratory Care
  • Assistive Technology: Tools and devices that help you stay independent at home
  • Personal Emergency Response System (PERS): Medical alert buttons in case of a fall or emergency
  • Non-Emergency Transportation: Medical and non-medical rides
  • Chore Services: Lawn mowing, snow plowing, window washing
  • Specialized Medical Equipment and Supplies
  • Counseling Services
  • Vehicle Modifications
  • Community Health Worker: Connects you to community resources and local support
  • Goods and Services: Things not covered anywhere else but essential to your care
  • Financial Management Services (Fiscal Intermediary): Handles payroll if you self-direct your care and hire your own caregiver

Note: MI Choice does NOT pay for room and board if you live in an adult foster care home or a home for the aged. It covers care services only — not housing costs.

How Do You Apply for the MI Choice Waiver? (Step by Step)

Applying can feel overwhelming. But it is much easier when you know exactly what is coming. Here is the full process, broken into simple steps.

Step 1 — Find Your Regional MI Choice Waiver Agency

Michigan has about 20 Waiver Agencies spread across the state, each serving a specific region. Many are run by Area Agencies on Aging (AAAs) or the Michigan Aging & Adult Services Agency. Your first step is finding the agency that covers your county.

Not sure where to start? Call Cottage Home Care MI at (313) 762-4272. We serve all 83 Michigan counties and can point you in the right direction.

Step 2 — The Phone Screening Call (About 30–45 Minutes)

Once you contact your local Waiver Agency, they will schedule a phone intake call. This is not the official assessment — it is just a first look. They will ask about your income, assets, and the type of care needed. If you seem eligible, they will add you to the waitlist.

Tip: Have this information ready before you call — monthly income sources (Social Security, pension), bank account balances, and a general idea of daily care needs. It will make the call faster and smoother.

Step 3 — The In-Home Assessment (LOCD — About 2 to 3 Hours)

When your name comes up on the waitlist, a Registered Nurse and a Social Worker visit your home. They use Michigan's official Level of Care Determination (LOCD) tool to assess your care needs. This visit usually takes 2 to 3 hours. Be honest and thorough — this determines how many care hours you receive.

Step 4 — Medicaid Financial Eligibility (MDHHS Decides)

The Michigan Department of Health and Human Services (MDHHS) makes the final call on whether you meet the financial requirements. You will need to gather documents and may need to apply for Medicaid through the MI Bridges portal if you are not already enrolled.

Here is what to have ready:

  • Social Security card and Medicare card
  • Recent bank statements — going back 60 months (5 years)
  • Proof of monthly income (Social Security, pension, retirement accounts)
  • Life insurance policy documents
  • Property deed for your home
  • Any pre-paid funeral contracts
  • Power of attorney documents, if someone is applying on your behalf

Missing documents are the number one reason applications get delayed. Gather these early.

Step 5 — Enrollment and Your Personal Care Plan

Once approved, you get assigned a Supports Coordinator — your personal care navigator. They build a care plan around your needs, your daily routine, and what matters most to you. Then services are set up and care begins. For more on what your rights and responsibilities are as a participant, download the official MI Choice Participant Handbook.

What About the Waitlist? Here Is What You Can Do Right Now.

Let us be honest about something most guides gloss over.

The MI Choice Waiver has a waitlist. Depending on where you live in Michigan, that wait can stretch from a few months to over a year.

Priority spots go to people currently in nursing homes who want to return to community living. For everyone else, your place in line is based on when you applied — not when you need care.

So what can you do while you wait? Quite a lot, actually.

  • Apply as early as possible. Your spot is locked in by your application date. Apply before you are in a crisis.
  • Explore Michigan's Home Help Program. This separate Medicaid program covers personal care tasks and may be available sooner.
  • Check out MI Health Link HCBS. If you qualify for both Medicaid and Medicare — called dual eligible — this program may offer immediate home care services.
  • Consider private pay home care as a bridge. You can start getting professional care at home right away while your application moves forward.

At Cottage Home Care MI, we offer private pay home care with no waitlist. Care can start quickly — sometimes within days. Explore our private pay home care options or call us at (313) 762-4272.

Can a Family Member Get Paid to Care for You Under MI Choice?

This might be the most-searched question in our entire MI Choice Waiver dedicated guide — and for good reason. The answer is yes, in many situations.

The MI Choice Waiver has a self-direction option. That means instead of being assigned a stranger, you choose your own caregiver — an adult child, a close friend, or even a trusted neighbor.

A Financial Management Services agency — also called a Fiscal Intermediary — handles the paperwork, background checks, payroll taxes, and payments on your behalf. You can learn more about how self-direction works in the official MI Choice Participant Handbook.

Here are the key rules:

  • The caregiver must be 18 or older
  • They must pass a criminal background check
  • A legal spouse cannot typically be paid through standard Medicaid Home Help rules (speak with your Supports Coordinator for MI Choice-specific rules)
  • Legal guardians and court-appointed financial managers are generally not allowed to also be the paid caregiver

This is one of Cottage Home Care MI's core specialties. We help families formalize the care that is already happening at home — turning informal help into a paid, legally compliant arrangement. We are Medicaid and CHAMPS certified, and we guide you through every step. Explore our Medicaid home care services.

MI Choice Waiver vs. Nursing Home: An Honest Look

Sometimes families feel like a nursing home is the only option. This MI Choice Waiver dedicated guide is here to show you there is another way. Here is an honest side-by-side comparison:

Factor MI Choice Waiver (Home Care) Nursing Home
Where you live Your own home or community setting Shared room in a facility
Who provides care A caregiver you choose Facility staff — assigned to you
Your cost Usually $0 (Medicaid pays) Room and board not covered by the waiver
Control over your day High — you set the schedule Low — facility runs the schedule
Cultural and language fit Yes — you can choose your caregiver Very limited options
Quality of life Home routines, family nearby Institutional setting
Waitlist Yes — limited slots available Usually immediate if a bed is open

For most families, staying home is not just more comfortable — research shows it supports better mental health and fewer hospitalizations. The MI Choice Waiver was built to make that possible.

MI Choice Waiver vs. the PACE Program: What Is the Difference?

Some people reading this MI Choice Waiver dedicated guide also want to know how MI Choice compares to the PACE Program (Program of All-Inclusive Care for the Elderly). Here is a quick breakdown:

Feature MI Choice Waiver PACE Program
Age requirement 65+ or disabled 18+ 55+ with nursing-home level need
Where care is delivered Home, adult foster care, community PACE center and home combined
Who runs your care You — with a Supports Coordinator A full PACE medical team
Coverage Long-term care services All Medicare + Medicaid services bundled
Self-direction option Yes — hire your own caregiver No — PACE team decides
How common in Michigan Statewide — 20 agencies Only in select locations

MI Choice is the better fit if you want to stay home, direct your own care, and choose your own caregiver. PACE may suit someone who wants one organization to manage all their healthcare in one place. Compare both on Benefits.gov to see which aligns with your situation.

Frequently Asked Questions About the MI Choice Waiver
How long is the MI Choice Waiver waiting list?
Wait times vary by region and change throughout the year. Some areas have shorter waits; others can take a year or more. Priority goes to people currently in nursing homes who want to return home. For everyone else, your place is based on when you applied — so apply as early as you possibly can.
How much does the MI Choice Waiver pay caregivers for seniors?
The program pays the Waiver Agency directly for authorized services. Most participants pay nothing out of pocket. If you self-direct your care and hire a family member, pay rates are set by the program and handled through a Fiscal Intermediary. Rates vary by service and region.
What is the MI Choice Waiver phone number?
To find the Waiver Agency in your area, visit the Michigan Department of Health and Human Services (MDHHS) website. You can also call Cottage Home Care MI at (313) 762-4272 — we will help connect you with the right agency for your county.
Is there a MI Choice Waiver application PDF I can download?
The MI Choice process starts with a phone intake call — not a downloadable form. Your regional Waiver Agency will walk you through completing the required state forms, including the DHS-390 and the MDHHS-6200 (filled out by your doctor). You can also apply for Medicaid through the MI Bridges portal if you are not already enrolled.
Which MI Choice Waiver agencies serve my county in Michigan?
There are about 20 Waiver Agencies across Michigan. Visit the Michigan Aging & Adult Services Agency website to find the one in your region, or call us and we will tell you exactly who to contact.
Can I apply for MI Choice Waiver online?
The initial intake screening happens by phone. However, the Medicaid financial application can be completed through the MI Bridges portal. Your Waiver Agency will guide you through which parts to do online and which require in-person or phone contact.
What happens if I am denied for MI Choice?
You have the right to appeal. Your regional Waiver Agency can help you through the appeals process. In the meantime, explore the MDHHS Home Help Program or consider private pay home care as a temporary option. A denial is not always the end of the road.
What is the difference between MI Choice and regular Medicaid home care?
Standard Medicaid home care — called the Home Help Program — covers basic personal care tasks like bathing and dressing. The MI Choice Waiver goes much further. It adds case management, home modifications, transportation, adult day care, meal delivery, respite care, and much more. It is designed for people who are seriously at risk of nursing home placement.
You Deserve to Stay Home. Let's Make That Happen.

Remember the moment at the beginning of this guide — when everything felt uncertain and the only question was: is there actually a way through this?

There is. And you just read it.

This MI Choice Waiver dedicated guide was written for every family sitting at a kitchen table at midnight, wondering what comes next. The answer is not always a nursing home. Sometimes, it is a program, a plan, and a phone call.

Here is your simple action plan for today:

  1. Check your eligibility — age, medical need, 2026 income limit ($2,982/month), and asset limit ($9,950)
  2. Gather your documents now — bank statements, income proof, Medicare card, and property deed
  3. Contact your regional MI Choice Waiver Agency to get on the waitlist as early as possible
  4. While you wait — explore the Home Help Program or private pay care for immediate support
  5. Call Cottage Home Care MI — we will help you navigate every step, in your language, at your pace

Home is not just a place. It is where your routines live. Where your memories are. Where your people come to find you. You should get to stay there for as long as you want.

This is such a helpful topic—thank you for covering it. When touring memory care centers for the first time, it can feel overwhelming with so many options. Is there a simple checklist or key things you recommend paying attention to during visits to help families make a more confident decision?

This was really eye-opening—especially the part about how the home environment can influence behavior and mood. We’ve been trying to figure out why evenings have been harder for my mother, and it never crossed our minds that lighting might be playing a role. Definitely gives us something practical to try.
